Learner Licence - Forms Required :

                             

                         1.   FORM NO.1 (Medical Certificate from MBBS Doctor) with self photo attested by the doctor with   

                               blood Group
                         2.   FORM NO. 2 (Application form of Learner License) Self Photo

3.   Residence Proof (Attested copy of) :

                                       a)  Ration Card OR
                                       b)  Voter’s Identity Card OR
                                       c)  Certified Copy of Voter List OR
                                       d)  Life Insurance Policy at least six months old with Latest Receipt
                                       e)  Valid Passport OR
                                       f)   Employer’s certificate in case of Central Govt./State Govt./Local  bodies supported by

                                            attested copy of Identity Card.                             

                          4.   Age Proof ( Attested copy of ) :
 

                                      a)  Educational Certificate OR
                                      b)  Birth certificate issued by Registrar Birth and Death showing the name of the applicant OR
                                      c)  Life Insurance Policy at least six months old OR
                                      d)  Valid Passport OR
                                      e)  Employer’s certificate in case of Central Govt./State Govt./Local bodies supported by

                                           attested copy of Identity Card

                                      f)   Certificate granted by registered medical practitioner, not below the rank of civil surgeon,

                                            as to the age of  the applicant.

                                     g)   Driving License issued by RLA, UT. Chandigarh (for Date of Birth only)

Address Proof :
                1.  Telephone Bill (BSNL Landline Only).

                2.  Certificate issued by the Hostel Wardens in case of students residing in Hostels.

                3.  Marriage Certificate issued by the Register of marriages, Chandigarh.

                4.  Residence Certificate issued by the Chandigarh Administration.

                5.  Rent Deed/ Rent agreement Registered with Sub-Registered of Chandigarh.

                6.  Police Report from the concerned S.H.O.

                7.  House Allotment Letter issued by Chandigarh Administration.

                8.  Ownership Letter issued by Estate Office/Chandigarh Housing Board.

                9.  Certificate of Registration of Firm (for RC only).
               10.  Electricity Charges Bill.
               11.  Water Charges Bill.
               12.  Sr. Citizen Identity Card issued by Social Welfare Department U.T., Chandigarh.
               13.  Identity Card issued to the Ex-serviceman, by the Zila Sanik Board.

                1.   Pan Card.
                2.   Driving License issued by the RLA, U.T.,Chandigarh OR
                3.   Certificate of age confirmation in case of illiterate person from Medical Officer of Government Dispensary
 

REGULAR LICENSE -

                         2.   FORM NO. 4

                         3.   Learner License duly passed by the motor vehicle inspector or D.S.P traffic

 DUPLICATE LICENSE - 

Documents Required :

                         1.   Self Photograph
                         2.   LLD Form
                         3.   Copy of DDR (Showing the license number)
                         4.   Blood Group in case not mentioned in the last License.

RENEWAL OF DRIVING LICENSE -

Forms Required :

                         1.  FORM NO.9
                         2.  FORM NO.1 (Medical certificate from MBBS Doctor) with blood group
 

Enclosure :
                         1.  Residence Proof :
                                      a)  Attested copy Voter card OR
                                      b)  Valid passport OR
                                      c)  LIC cover note at least six months old with latest receipt of payment OR
                                      d)  Employer certificate issued by the competent authority with I-Card in case of employee of state

                                           or central govt. OR
                                      e)  If an applicant does not have any of the proof mentioned above he may attach copy of Ration

                                           Card with affidavit.
